Get Your Seasonal Flu Shot and other vaccinations with Utah State Eastern’s 2nd-year AAS/RN student nurses. Protect yourself, your loved ones, and our community! Everyone is welcome! Students, Faculty, Staff, families, and community members, we will meet you at the Jennifer Leavitt Student Center Hallway. ✅Pre-Register: seuhealth.com/pre-register/ Please upload your insurance card when completing the form. If you do not pre-register, you must have your insurance card or pay cash. Bring your Immunization Record/Card (if available). 💉 Vaccines Available: • Flu Shot, HPV, Meningitis (A, C, W, Y & B), Adult RSV 🎉 FREE Flu Shots for the first 300 Utah State Eastern Students, Staff, and Faculty (must provide A-number). 💲Cash Pay: $35 Regular Flu Shot | $85 High-Dose 🩺The 2025 Flu Shot Clinic is presented by Utah State Eastern in conjunction with the Southeast Utah Health Department and USU Nursing.
