How to Enroll and Make Changes
Medical Plan
Including healthcare and/or dependent care, flexible spending account (FSA), and vision.
- Visit pehp.org.
Dental Plan
- Complete the EMI Enrollment Application/Change Form.
Life Insurance and Short-Term Disability Insurance
- Beginning July 1, 2024, USU will have a new provider, The Standard, for short-term disability, long-term disability, and life insurance
- Complete the Standard Enrollment Form
MetLife Legal
- Complete the MetLife Legal Plans Enrollment Form.
