Hours: Monday-Thursday, 9:00 - 11:00 am and 2:00 - 4:00 pm, closed Fridays, weekends and holidays.

This card has $2.10 to pay for printing every semester, once all payments to the school are completed.

On campus and around town, you may get a discount with your card. This includes a few visits to Urgent Care as per USU Eastern Health Services.
Card Information
Who qualifies for a USU Eastern ID card?
- Currently enrolled student
- Concurrent enrollment students (with a fee)
- Concurrent enrollment student cards to do not have all the same abilities. It does still work as a library card or for discounts.
- Current employee of USU Eastern
- Employee ID cards give access to library collections and the BDAC.
- In most cases, your card works as a key for building spaces where you work. To have access added to your card, have your building Key Guard or your department head submit a key request.
How do I get my ID card?
- Go to the Eastern ID Card Office at the Library Learning Commons (LLC).
- Provide one form of government ID
- For example: driver's license, state-issued ID, passport
- For a concurrent enrollment student or replacement card, be prepared to pay a $15 fee cash or card.
How do I get my photo?
- Let us take your photo. Come to the LLC, and we will take your photo when you get your card.
- or- - Take your own photo and upload it to Banner.
- Photo guidelines:
no sunglasses (regular glasses are ok); no hats, face, or head coverings; (exceptions for religious or medical reasons)
How do I report my card lost or missing?
- Go to Manage My Card
- Follow instructions for deactivating card.
This will freeze all funds, meals, and access to buildings.
Do I qualify for specialty card?
- All specialty cards require that the department or your instructor contacts the Card Office ahead of time. They will provide a list of approved persons allowed to be given a specialty card.
- Some departments pay for your card ahead of time, and some require you to pay the Card Office directly. As your instructor for clarification about payment.
- Nursing cards are given to instructors (do not pick up your card from the Card Office).
- Aviation cards require that you wear your official embroidered aviation shirt for the picture.
- TEAL cards require you to bring the instructor given signed paper work to the Card Office.
- Official USU Safety/Police cards require either the USU Eastern Police Chief to contact the Card Office ahead of time, or for you to have a USU Police Office accompany you when you come.
